How can a LAC help you?
Our Local Area Coordinators are here to assist on your pathway with the NDIS.
Feros Care has partnered with the National Disability Insurance Scheme (NDIS) to help people with disability live healthy, fulfilling and connected lives.
A Feros Care Local Area Coordinator (LAC) will work with you to understand what is important to you and connect you to the services that will support you to live the life you want. In a nutshell, they will help you move from stage to stage on your pathway with the NDIS.
LACs can meet with you, your family and carer to help you:
- understand the NDIS
- link you to information and community supports
- assist you with access to the NDIS
- work with you to create a First Plan based on your goals and current supports
- support you to put your NDIS plan into action and to review your plan
Your support services will all be tailored to meet your goals and improve your overall wellbeing.
You do not have to pay an LAC for any of the support they provide to you.
Funding does not need to be included in your NDIS plan for any LAC services.
How does the NDIS work?
Access
Your LAC can help you to request access to the NDIS. They will discuss your existing supports, your needs and your goals.
Approval
When your NDIS plan is approved, you will work with your LAC to put it into action.
Choose
Your LAC will help you connect with and choose the supports needed to help you to achieve the goals set out in your plan.
Contact
Your LAC is your NDIS contact person to discuss any questions regarding your NDIS plan.
Review
Your LAC is available to discuss the progress of your plan with you and your family, and any other options to be considered as you review and develop your plan.
As well as providing Local Area Coordination services for individuals, Feros Care is actively working with the local community to provide information, linkages and support projects for people with disability. What we do depends on the needs that are identified in each of the communities we work in.
This could be working with mainstream services like GPs and health care professionals to support them to understand the NDIS and make the process of gaining access to the NDIS smoother for their clients.
We also connect with schools and employment services to provide information and support to school leavers with disability. If you see a need in your community where Feros Care might be able to assist, please feel free to chat with your LAC about it.
